29 CFR 1915.111(a): 2 - Defective Gear was not removed and repaired or replaced before further use.      a) Crounse Corp., Maysville, KY - On or about 1/7/15, employees were exposed to struck-by hazards when hooks on the jib crane was operated with a damaged safety latch.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).    Crounse Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1915.111(a), which was contained in OSHA inspection number 301511176, citation number 1, item number 2 and was affirmed as a final order on 12/9/2010, with respect to a workplace located at 9104 Old Lock and Dam Rd, Maysville, KY 41056.
