(a) Shore Side Workshop, vicinity of 400 Marine Way, Paducah, KY: On or about 3/07/2024, and at times prior, the employer exposed employees to severe laceration injuries, in that the safety flange was improperly mounted on a 4.5-inch Milwaukee grinder, model 6130-33, which positioned the grinding disc outside of the guard while employees were grinding metal parts.
