29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered.  a) Argon Oxygen Decarburization (AOD) Vessel, 500 East Centre Street, Ashland, PA - During the cleaning of an exhaust hood above the AOD vessel, an employee stood on the upright AOD vessel and used a metal pry bar to loosen slag that had splashed onto the exhaust hood. An employee exposed to a fall hazard was not provided protective equipment, on or about January 9, 2015.  In accordance with 29 CFR 1903.19(c), abatement certification is not required for this violation. The hazardous condition was corrected during the inspection.
