OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and fall hazards created by the operation and placement of the argon oxygen decarburization (AOD) feed hopper. a) Melting Deck, 500 East Centre Street, Ashland, PA - Employees manually placed raw materials into the argon oxygen decarburization feed hopper that was below floor level, was controlled from a remote location (AOD Control Room), and had an unprotected opening. Employees were exposed to struck-by and fall hazards, on or about January 15, 2015. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

Bench Grinder Model No. 500 (Serial No. 598165) that had an opening between the work rest and outer periphery of the abrasive wheel on the left that was one and three quarter-inches (1 3/4") and an opening between the work rest and outer periphery of the abrasive wheel on the right that was one and one quarter-inches (1 1/4"). Employees were exposed to caught-in hazards on or about January 20, 2015. In accordance with 29 CFR 1903.19(c), abatement certification is not required for this violation. The hazardous conditions were corrected during the inspection.
