29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:        a)    On or about 1/22/2015 the Seiko Die Cutter had an unguarded opening where the die cutter comes down on the material being processed. The nip point was approximately 21" from the control panel and approximately 8" from trimming hexagon shaft.    b)    On or about 2/12/2015 the #1 laminator glue rolls needed better nip point guarding. It was approximately 12" from the frame of the machine to the ingoing nip point on the glue rolls.
