29 CFR 1910.132(d)(2):The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment. At the establishment located at 352 6th Street, Menasha, Wisconsin; the employer did not certify that a workplace hazard assessment at the Dura-Wrap Section had been performed through a written certification that identified the workplace evaluated; the person certifying that the evaluation had been performed; and the date(s) of the hazard assessment. Employees were required to wear PPE, such as, but not limited to safety glasses, protective footwear, and earplugs when exposed to hazards, such as but not limited to struck by flying debris, excessive noise, and caught under object.
