29 CFR 1910.23(d)(1)(iii): On stairways less than 44 inches wide having both sides open, one stair railing on each side.  a) On or about Monday, January 26, 2015, the open stairway, leading the RVR Building Platform, was equipped with only one handrail. Employees were exposed to falls from elevation which could cause broken bones, fractures, and possibly even death.

29 CFR 1910.23(e)(1): A standard railing shall consist of top rail, intermediate rail, and posts, and shall have a vertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level. The top rail shall be smooth-surfaced throughout the length of the railing. The intermediate rail shall be approximately halfway between the top rail and the floor, platform, runway, or ramp. The ends of the rails shall not overhang the terminal posts except where such overhang does not constitute a projection hazard.  a) On or about Monday, January 26, 2015, the open stairway, leading to the RVR Building Platform, was not equipped with an intermediate rail. Employees were exposed to falls from elevation which could cause broken bones, fractures, and possibly even death.

29 CFR 1910.303(b)(7)(iv): There shall be no damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating.  a) On or about Monday, January 26, 2015, in the HUF Air Surveillance Radar Room, a burned electrical receptacle was being utilized as a power source. The employees were exposed to electrical burns, electric shock or electrocution.

Live electrical parts were exposed due to a missing knockout approximately 1 in diameter.  a) On or about Monday, January 26, 2015, on the north wall of the supervisors office, a knockout was missing in the electrical box. The missing knockout exposed employees to live parts which could result in electrical burns, electric shock and possible death by electrocution.

29 CFR 1910.305(g)(1)(iv)(B): Flexible cords and/or cables were run through holes in walls, ceilings, or floors:  a) On or about Monday January 26, 2015, in the Flight Service Station building, an electrical calibration cable was running through a 3 inch hole that was cut in an existing door that lead into a mechanical room. The pinched cable exposed employees to possible electrical burns, electrical shock and death by electrocution.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.  a) On or about Monday, January 26, 2015, in the break room,  the carpet was totally separating across the floor.  Employees were exposed to slips, trips and falls which could result in bruises, cuts and broken bones.
