OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught-in and crushed-by hazards related to the operation of under hung cranes: a) Packaging Department - The employer did not ensure that the bowl of the hook was seated in the load, sling or lifting device while operating an under hung crane. Employees were exposed to caught-in and crushed-by injuries on or about January 6, 2015. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.184(c)(10): Hands or fingers shall not be placed between the sling and its load while the sling is being tightened around the load. a) Packaging Department - An employee's hand and/or fingers was placed between the load and a chain sling while the sling was being tightened. When one of the lifting hooks disengaged the employee's hand became caught in between the load and the sling resulting in a fingertip amputation, on or about January 6, 2015. No abatement certification or documentation required.
