29 CFR 1910.212(a)(3)(ii): The point of operation machine guards were not designed or constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    a)  Enterprise 18-inch Alligator Shear #2 (Model 200/18-RH, SN 5439).  On or about January 13, 2015, the operator(s) were able to reach under the cage guard and put their hands in the point of operation while placing materials for cutting, exposing operator(s) to amputation and laceration hazards.         b)  Enterprise 18-inch Alligator Shear #2, Linemaster Foot Pedal Control (Full Twin Guard Cat. 522-D28.)  On or about January 13, 2015, the operator(s) used a foot pedal to initiate the shearing action of the blade.  The foot pedal control was not guarded to prevent inadvertent activation, exposing operator(s) to amputation and laceration hazards.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).     CAI CUSTOM ALLOYS, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 92082 CITATION NUMBER 1 ITEM NUMBER 2 AND WAS AFFIRMED AS A FINAL ORDER ON NOVEMBER 18, 2011 WITH RESPECT TO A WORKPLACE AT 6871 BELFORD INDUSTRIAL DRIVE, BELVIDERE, ILLINOIS, 61008.
