29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about June 18, 2018, the operator(s) were able to reach under the cage guard and put their hands in the point of operation while placing materials for cutting, exposing operator(s) to amputation, laceration, and fracture hazards on the Gensco DTX 600 Shear. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d) The CAI Custom Alloys, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1910.212(a)(3)(ii)), which was contained in OSHA inspection number 1020204, citation number 1, item number 1 and was affirmed as a final order on 07/23/2015, with respect to a workplace located at 1462 US Route 20, Belvidere, IL 61008.
