29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used: a. On or about January 22, 2015 - at a jobsite located at 700 W. Lincoln Highway, Chicago Heights, IL. - Employees performing tear off procedures from ladderjack scaffolding were not provided an additional ladder to safely access the scaffold platform, thereby exposing the employees to fall hazards of approximately twelve (12) feet.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system: a. On or about January 22, 2015 - at a jobsite located at 700 W. Lincoln Highway, Chicago Heights, IL. - Employees were performing tear off activities from ladderjack scaffolding without the use of personal fall arrest systems, thereby exposing the employees to fall hazards of approximately twelve (12) feet.
