On or about April 11, 2019, at the above addressed jobsite, the electrical box cover or face plate was removed to power the swing stage scaffolding on the second floor and was to protected against accidental contact, thereby exposing employees and subcontractor employees to electrical hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. On or about April 11, 2019, at the above addressed jobsite, an employee removing brick from a parapet wall on the roof, approximately forty (40) feet above the ground, was exposed to fall hazards created by the presence of unprotected sides. Dardon's Roofing, LTD. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1126107, citation number 1, item number 2 and was affirmed as a final order on May 16, 2016, with respect to a workplace located at 3863 N. Drake Ave., Chicago, IL.
