29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a) On February 18, 2016, employees working from a ladder jack scaffold were exposed to a fall hazard of approximately 21.8 feet. Dardon's Roofing, LTD. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1)(i), which was contained in OSHA inspection number 1020367, citation number 1, item number 2 and was affirmed as a final order on April 15, 2015, with respect to a workplace located at 700 W. Lincoln Hwy., Chicago Heights, IL, 60411. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records. 29 CFR 1926.451(e)(1): Safe access was not provided to scaffold platforms when the platforms were more than 2 feet above or below the point of access: a) On February 18, 2016, employees were exposed to a fall hazard of approximately 21.8 feet while accessing a ladder jack scaffold.
