29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about January 22, 2015 and times prior, employees were not protected from injuries while operating and working around the tripper on the gallery conveyor system where there was an unguarded in-running nip point on the head drum of the tripper. Abatement certification is required for this item.

Located in the west unit bathroom area was an energized conductor (wire) that was not capped and or rendered de-energized.
