29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D)(h): a/ Bowlmor AMF DBA/Leiserv, LLC Brunswick Zone Carolier North Brunswick, NJ Site : Bowling Machines Pin-Setting/Bowling Pad Area: The lockout program provided by the employer did not include, but not limited to: 1/ the program was not documented to show the limitation of service to be provided by the C-Technician/Pinsetter and the maintenance mechanics 2/ the program did not have specific steps documented to ensure the equipment was adequately de-energized prior to being serviced or repaired 3/ means to control the use and identity of locking devices 4/ designated personnel involved in the program, etc. Equipment include: Bowling Machines and associated electrical equipment. Observed on or about 01/28/2015.

On/or about 01/23/2015 the employer did not ensure that the employee performing the pad changing operation used locking devices during that operation. Observed on or about 01/28/2015.
