29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level:      On or about March 08, 2016, employees tasked with servicing/maintaining the forty-eight (48) pin setting machines, located in the machine area of the bowling center, were not protected from a fall hazard more than forty-eight (48) inches because the corresponding hand rails were missing mid-rails.

29 CFR 1910.26(c)(1): Good safe practices in the use and care of ladder equipment were not employed by the users to get maximum serviceability, safety, and to eliminate unnecessary damage of equipment:          On or about March 08, 2016, while in the process of troubleshooting lane #37 an employee; who had been using a portable 3 ft. metal step ladders to ascend and descend the pin setting equipment, fell when the step ladder (which was not fully extended) kicked back/out and caused the ladder to tip over. The employer failed to ensure that safe work practices in the use of portable metal step ladders were established; implemented and their use enforced in that, the hardware connections for the ladders was not inspected prior to each use creating a fall hazard.

29 CFR 1910.37(a)(3):  Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level:      On or about March 08,2016, the doorway threshold of the exit discharge was not provided with stairs and/or a ramp. Employees who use the exit route when receiving deliveries and/or as an exit discharge in the event of an emergency are exposed to potential trip/fall hazards of approximately 15.5 - 16 inches.

29 CFR 1910.219(i)(1): Screw(s) or bolt(s) used in revolving collar(s) projected beyond the largest periphery of the collar:       On or about March 08, 2016, the employer failed to ensure that the pin distributor on lanes including; but not limited to, lane #37 were equipped with a flush screw/bolt/pin in the revolving collar of the distributor clutch mechanism. An employee in the process of troubleshooting the machinery was injured when his finger inadvertently got caught by the head of an Allen head capped screw on the rotating/revolving collar.
