OSH ACT of 1970 Section (5)(a)(1):     Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employee(s) in that employee(s) were exposed to the hazard of being caught-in:    Inside of the Fabrication Shop, employees were exposed to an amputation/laceration hazard due to a possible machine process malfunction on the following equipment:    a/  Press Brake #1, unknown make, model, or serial number.    One method of abatement to correct this apparent hazard is to install emergency stop devices as required by ANSI B11.3-2002, paragraph 6.1.3.1.    DISCLAIMER:    a/  The employer is not limited to the abatement method(s) suggested by OSHA.  b/  The employer is responsible for selecting and implementing an effective abatement method.

29 CFR 1910.212(a)(3)(ii): The point of operation (P.O.O.) of machines whose operation exposes an employee to injury, was not guarded. The guarding device was not in conformity with any appropriate standards therefore, or, in the absence of applicable specific standards, was not designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle:    a/  Inside of the Weaving Department, the P.O.O. belonging to the Off-Line Crimper Machine was inadequately guarded. The barrier guard had an opening around the P.O.O. that measured approximately two inches wide by five inches high.    b/  Inside of the Weaving Department, the P.O.O. belonging to the Loom #74 was inadequately guarded.  The barrier guard had an opening in front of the P.O.O. that measured approximately 1.5 inches above the table.      c/  Inside of the Fabrication Department, the P.O.O. belonging to the Press Brake #1 was unguarded in the front, sides, and rear of this machine.

29 CFR 1910.212(a)(1):  One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    Inside of the Weaving Department, the following machines were not adequately guarded:    a/  Off-Line Crimper Machine, unguarded nip points that guide the wire into the machine.    b/  P-4 Crimping Machine, unguarded nip points that guide the wire into the machine.    c/  Loom #72, unguarded "Beater" that generates a reciprocating motion.    d/  Loom #74, unguarded "Beater" that generates a reciprocating motion.      METSO MINERALS INDUSTRIES INC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 904534, citation 1, item 1 and was affirmed as a final order on 12/31/13, with respect to a workplace located at 1402 E. Veterans Memorial Parkway, Warrenton, MO 63383.
