29 CFR 1910.22(d)(1): (1)     Walking-working surfaces were not inspected, regularly and as necessary, and maintained in a safe condition.    On or about 11/28/2018, Metso Minerals Industries Inc. did not adequately inspect and maintain the employees walking-working surfaces.  Inside of the synthetic department, a Carlton Radial Arm Drill was used to fabricate parts.  The base of the drill extended beyond the drill press table.  The employees were required to stand on the base to operate the radial arm drill.  The base was not a safe walking-working surface.

29 CFR 1910.147(d)(2):     Machine or equipment shutdown.  The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment.    On or about 11/28/2018, employees at Metso Minerals Industries Inc. failed to de-energize the Allen Drill Press, no identifiable serial or model numbers, located in the maintenance department.   The drill press and lubrication control/device had the following hazardous conditions:  a)  The shroud/guard designed enclose the pulleys and belts was missing.  b)  The electrical cover/panel was not attached to the Kool Mist Electrical Box.

29 CFR 1910.212(a)(3)(ii):     The point of operation of machines whose operation exposed an employee to injury, was not guarded. The guarding device was not designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    On or about 11/28/2018, employees from Metso Minerals Industries Inc. were exposed to hazards such as those created by point of operation.    The following equipment did not have guarding devices designed and constructed as to prevent the operators from having any part of their bodies in the danger zone during the operating cycle:  a)  An Allen Drill Press in the maintenance department, no identifiable serial or model number, was inadequately guarded.   The guard was did not fully enclosed the chuck and drill bit during operation.  b)  A Carlton Radial Arm Drill in the synthetic department, no identifiable serial or model number, was inadequately guarded.   The guard was did not fully enclosed the chuck and drill bit during operation.    Metso Minerals Industries Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(iii) which was contained in OSHA inspection number 1021286, citation number 1, item number 5, issued on 4/02/2015, and was affirmed as a final order on 4/27/2018, with respect to a workplace located at 1402 E. Veterans Memorial Parkway, Truesdale, MO 63380.
