No materials or equipment may be placed, either permanently or temporarily, within the exit route:  a) Facility, 1924 Puritan Road, Portage, PA:  On or about January 28, 2015, the access and egress to the employees work area on the catwalk was covered in magazines on the floor at the bottom of the steps, on the steps, and piled up next to the steps so as to provide a 21 inch opening at the stair entrance.

If such hazards are present, or likely to be present, the employer shall:    a) Facility, 1926 Puritan Road, Portage, PA:  On or about January 28, 2015, employees were exposed to potential hazards because the employer did not complete a workplace PPE assessment to determine what work activities needed PPE and what type of PPE would protect the employees during those activities, such as, but not limited to, working on the conveyor belt which loud and would necessitate the need for ear protection.

TR-10-75 without a lockout procedure or training to properly lock out the equipment.    b)  Nings Magnet, facility, 1926 Puritan Road, Portage, PA:  On or about January 28, 2015, an employee was exposed to potential hazards while greasing the fittings on the magnet serial no. K9413 without a lockout procedure or training in to properly lock out the equipment.
