OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards and falling loads due to not properly latching in the containers and from cables and hooks used during hoisting operations that were not in good condition:    (a) Jobsite- 188 Gilbert Street, Johnstown, PA- On or about July 2, 2018, the Heil Rear Loader W6 latch to secure the containers to the truck was non-functioning.    (b) Jobsite- 188 Gilbert Street, Johnstown, PA- On or about July 2, 2018, employees operating Heil Rear Loader W6 did not use the trucks latches to secure the containers to the truck.    (c) Storage Yard- 159 Tire Hill Road, Johnstown, PA- On or about July 6, 2018, the wire cable for the winch that lifted the containers was observed frayed and in poor condition.     (d) Storage Yard- 159 Tire Hill Road, Johnstown, PA- On or about July 6, 2018, the hook used to lift containers was observed missing the latch.
