29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed: a) On or about 01/28/15 in the Injection Mold machine area; employer failed to protect employees from caught between hazards during service and maintenance activities, such as but not limited, to changing molds on the injection mold machines when they did not conduct an initial or periodic inspection of the energy control procedure. b) On or about 01/28/15 in the RIM area; employer failed to protect employees from caught between hazards during service and maintenance activities, such as but not limited to, changing molds on the RIM machines, when they did not conduct an initial or periodic inspection of the control of hazardous energy procedure. NO ABATEMENT CERTIFICATION REQUIRED
