29 CFR 1910.243(c)(5)(i): The spindle speed of the machine was not checked before the mounting of the wheel to be certain that it does not exceed the maximum operating speed marked on the wheel. (a) On or about December 30, 2014, a 30,000 RPM grinder was used and exceeded the operating speed of a 15,279 RPM grinding disk, exposing employees to struck-by hazards.
