Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems.  On 1/28/2015, workers from Builders Bloc Contracting Services LLC were installing boards above a window on the second story of a residential building located on Lot 11 on Distinction Drive in Lake St. Louis MO.  One employee was working of the roof and on a ladder, while positioned on the roof, without fall protection, exposing him to fall to the ground below.  He was not utilizing guardrail systems, a safety net system, or personal fall arrest systems.
