29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a. On or about January 02, 2015, at the workplace, the employer did not ensure that, when an employee fell from the third step of a step ladder, and had to be sent to the hospital, the incident was reported within 24 hours after an injured employee was admitted into the hospital. b. On or about January 03, 2015, at the workplace, the employer did not ensure that, when an employee slip on ice and hit his head, and had to be sent to the hospital, the incident was reported within 24 hours after an injured employee was admitted into the hospital.
