29 CFR 1910.147(d)(4)(ii): Lockout devices utilized were not affixed in a manner that held the energy isolating device in a safe or off position. On or about January 22, 2015, employees were exposed to amputation hazards on a Phlauer Mixer during testing and the employer failed to ensure a lockout device was affixed prior to work activities. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(f)(1): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, equipment or component thereof, the sequence of actions were not followed in accordance with 29 CFR 1910.147(f)(1): On or about January 22, 2015, employees were exposed to an amputation on a Phlauer Mixer during testing and the employer failed to follow the requirements under section (f)(1). Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
