29 CFR  1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):  On or about April 4, 2024, the employer did not ensure that the machine known as the conti-mixer was isolated from the energy source and rendered inoperative prior to an employee performing servicing and maintenance. The employee was exposed to rotating parts while the machine was running, while it was being cleaned, which led to an injury.  Alternate protective measures such as machine guarding were not provided at the time of the accident.   No abatement documentation or certification is required for this item.
