29 CFR 1904.29(b)(3): How quickly must each injury or illness be recorded? You must enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred. (a): On or about March 29, 2023, an OSHA Form 301 or equivalent was not completed by the employer for a work-related injury within seven calendar days. An employee was injured and subsequently hospitalized after being caught between a Crown End Control Pallet Truck and guard rail. (b): On or about March 29, 2023, a work-related injury that resulted in an employee hospitalization was not recorded on the OSHA 300 log within seven calendar days. An employee was injured and subsequently hospitalized after being caught between a Crown End Control Pallet Truck and guard rail. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.178(l)(6):Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. (a): On or about March 22, 2023, the employer did not certify that each operator had been trained and evaluated as required. The employer did not have certification for an employee who operated the Crown End Control Pallet Truck. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
