29 CFR 1926.1053(b)(4): Ladders shall be used only for the purpose for which they were designed: a) On or about January 30, 2015 - at a jobsite located at 37th & Saginaw Avenue, Chicago, IL. - an employee performing carpentry operations on a roof was using a step ladder in a manner other than designed, thereby exposing the employee to fall hazards of approximately twelve (12) feet.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502: a) On or about January 30, 2015 - at a jobsite located at 37th & Saginaw Avenue, Chicago, IL. - employees performing carpentry operations on a roof were exposed to fall hazards of approximately twelve (12) to twenty (20) feet to the lower levels created by the presence of unprotected sides and edges. The ACC, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 702598, citation number 1, item number 1 and was affirmed as a final order on December 05, 2012 with respect to a workplace located at 945 W. 36th Street, Chicago, IL. 60609.
