29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2):   Maintenance - Employees have been provided and are required to wear personal protective equipment such as eyewear, earmuffs and ear plugs, gloves, and dust masks to protect themselves against hazards such as flying particles and dust, chemical irritants, and hearing loss.  A hazard assessment has not been performed to include the following elements:          1.  Select and have each affected employee use the types of PPE that will protect them from the hazards identified in the hazard assessment;  2.  Communicate the selection decisions to each affected employee and;  3.  Select PPE that properly fits each affected employee.          *ABATEMENT NOTE:  The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and which identifies the document as a certification of hazard assessment.
