On February 4, 2015, near the rear side of the house in the interior, a metal electrical box, designed to be mounted in a wall was used as an extension cord outlet that supplied power for a Rigid Drill.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502: a. On February 4, 2015, on the front side of the house, there was no fall protection for an employee painting primer on the roof and parapet around the perimeter of the roof, exposing employees to a fall from a height of about 14 feet, 2 inches.
