29 CFR 1910.254(a)(3): Workmen designated to operate arc welding equipment had not been properly instructed and qualified to operate such equipment as specified in 29 CFR 1910.254(d) Welding areas: on or about February 2, 2015, employees assigned to operate arc welding equipment to fabricate steel frames and tanks were not properly instructed and qualified in safe operation and use of the equipment. Employees were exposed to explosion struck-by and electrical hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.254(d)(1): Workmen assigned to operate or maintain arc welding equipment were not acquainted with the requirements of 29 CFR 1910.254(d) and 29 CFR 1910.252(a), (b), and (c) Fabrication, welding areas: on or about February 2, 2015, employees assigned to operate arc welding equipment to fabricate steel frames and tanks were not properly instructed and qualified in safe operation and use of such equipment as required by 1910.254(d) and were not acquainted with the requirements of 1910.252(a), (b), and (c); including, fire prevention, personnel protection, health protection and ventilation. Employees were exposed to explosion, struck-by and electrical hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
