OSH ACT of 1970 Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing, or likely to cause, death or serious physical harm to employees in that employees were exposed to falls through grating serving as floor of runways and/or catwalks:     Employees working on the elevated deck/walkway at the 11th floor level of the regenerator, at FCCU#2 at the 30Block, walked on sections of grated flooring which had the following hazardous conditions and exposed employees to potential falls through the metal grating of the walkways:     A. The design of a section on the 11th floor of FCCU#2 was not sufficient to prevent shifting of the individual sections of the walkway/catwalk.    B. #9 wire or similar wiring was used to permanently secure grated sections together instead of appropriate clips or saddles.

29 CFR 1910.36(g)(1): The ceiling of an exit route must be at least seven feet six inches (2.3 m) high. Any projection from the ceiling must not reach a point less than six feet eight inches (2.0 m) from the floor.   On January 13, 2015, and times prior thereto, at the FCCU#2, Reactor/Regenerator Structure, exit routes were obstructed by insulated, heated piping. The piping is permanently protruding above the walking/ working surface, at chest to head level.
