29 CFR 1910.28(b)(3)(iv):  Each employee was not protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset was not used.     a) On or about 04/18/2018, at Sulfur Recovery Unit (SRU) within CITGO West Plant facility located in Corpus Christi Texas, on the rooftop of the caustic tank (585-TK-6018: 20% NaOH Caustic), the employer allowed employees to conduct an inspection on the rooftop of the tank near an unguarded ladderway floor opening.    b) On or about June/13/2018, at #5 Pflatformer unit within CITGO West Plant facility located in Corpus Christi Texas, on the top of a platform which is closed to a booster compressor jacket water expansion tank, the employer allowed employees to check a level on the booster compressor jacket water expansion tank near an unguarded ladderway floor opening.

High Level Alarm LH-257 (P&ID 546-4-8110A Rev.8)   b. Level Alarm L-122 (P&ID 546-4-8110A Rev.8)   c. Level Alarm L-150 (P&ID 546-4-8112B Rev.16)   d. Pressure Alarm PI-281(P&ID 546-4-8115B Rev.9)   e. Level Alarm L-204 (P&ID 546-4-8115B Rev.9)  f.  High Level Alarm LH-275 (P&ID 546-4-8129A Rev.9)    The CTIGO Petroleum Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.119(j)(4)(i),which was contained in OSHA inspection number 944145, citation number 01 item number 01 and was affirmed as a final order on April 13th 2016 with respect to a workplace located at 135th & New Avenue, Lemont, IL 60439
