29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    a)  On February 6, 2015, fall protection was not provided for employees receiving/unloading concrete cement blocks on the roof of a new home under construction.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support:    a) On February 6, 2015, a job-made wooden extension ladder, approximately 18 feet in length, was used at an angle greater than one-quarter the length of the ladder to access the roof.

29 CFR 1926.51(f)(1): The employer did not provide adequate washing facilities for employees engaged in the application of paints, coatings, herbicides, or insecticides, or in other operations where contaminants may be harmful to the employees.  Such facilities were not in near proximity to the worksite and was not equipped as to enable employees to remove such substances:   a) On February 6, 2015, there was no soap and no hand towels or other drying methods available in for employees that used Portland cement products in the process of laying concrete blocks and footings.
