29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, were not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  (a) At the project on or about March 27, 2015 and at times prior thereto, construction materials and debris were not kept clear as there were cans, empty bags of cement, broken concrete blocks, pieces of rebar, and nails all over the exterior and interior of the house.  Employees were exposed to a hazard of falling to the same level onto the debris and the materials.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not being protected by protective helmets:  (a) At the project on or about March 27, 2015 and at times prior thereto, inside of the house, head protection was not worn while working below employees carrying 2 by 4 rebars and hand tools to build formwork on the roof and the interior of a two-story concrete building, exposing employees to a hazard of being struck by construction equipment and materials.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not being protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    (a) At the project on or about March 27, 2015 and at times prior thereto, on the 2nd floor of the roof, there was no fall protection for employees that were building formwork, exposing employees to a 10-foot fall hazard.     (b) At the project on or about March 27, 2015 and at times prior thereto, on the 1st floor roof above the garage, there was no fall protection for employees walking and working less than 1 foot from the edge of the roof picking up rebars and 2 by 4 inch lumber, exposing employees to a 10-foot fall hazard.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:  (a) At the project on or about March 27, 2015 and at times prior thereto, from the 1st floor roof of the garage to the 2nd floor roof of the house, an eleven foot long job-made ladder did not extend at least 3 feet above the landing for the roof, exposing employees to a hazard of falling approximately 18 feet to the ground.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with one stairrail system along each unprotected side or edge:  (a) At the project on or about March 27, 2015 and at times prior thereto, near the front entrance side of the first level, there were no stairrail systems on the unprotected edges of a stairway with 14 risers to the first landing to the second level.  Employees used the stairs to access the second level and were exposed to a hazard of falling approximately 10 feet to the concrete floor.
