JOB SITE: 5300 Hall Street, Saint Louis, MO 63147, on or about 2/6/2015, at this facility, located in the tote wash station each outlet box in completed installations did not have a cover. The following outlet boxes were missing covers: 1) In the tote wash station, on the east wall, missing cover. 2) In the tote wash station, employees' break/lunch room, missing face plates.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: JOB SITE: 5300 Hall Street, Saint Louis, MO 63147, on or about 2/6/2015, at this facility, located in the tote wash station, an orange flexible cord was used as a substitute for fixed wiring that wrap over and about the tote wash station of a structure. The orange flexible cord was used to power a Ridgid Floor Fan, Model: AM 25001, S/N 05069...0153, Air Mover, 120V. The workers were exposed to a potential an electrical hazard.

The power cord jack or insulation was repaired with black electrical tape. The workers were exposed to a potential electrical hazard.
