29 CFR 1910.147(c)(1): Energy control program. The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment would be isolated from the energy source and rendered inoperative.  a) At the Plant - The employer did not develop and implement a Lockout/Tagout program for equipment such as but not limited to Eco-lite Machines, Rotary Cutters and Cast Line Plastic Extrusion Machines; exposing employees to electrocution and amputation hazards.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains were not enclosed unless they were more than seven (7) feet above the floor or platform. Where the drive extended over other machine or working areas, protection against falling was not provided.  a) At the 6 feet Cast Line Plastic Extruder Machine - The employer exposed employees to nip point, caught-in and amputation hazards by allowing employees to work near unguarded chains and sprockets from the rewrapping of trims.
