The company failed to conduct a yearly audit of their Lock out/Tag out program. An employee suffered an amputation to his right middle finger when reaching into a blender to clear a jam, on or about December 27, 2018. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
