Center St. Marion, OH: The employer failed to ensure that energy control procedures were utilized to control potentially hazardous energy sources while employees performed cleaning and die changing activities to the Cutter Machine on line #7202. Employees performing these activities in this condition are exposed to unexpected energization at the machines point of operation.

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) The employer failed to ensure that energy isolating procedures for shutdown were performed prior to conducting cleaning and die changing work on the cutting machine on Line #7202. Employees performing these tasks in an energized condition were exposed to amputation hazards at the machine's point of operation, in that energy control shutdown procedures required under 29 CRF 1910.147(d)(2) were not utilized before the employee began the aforementioned work task(s) on the machine.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Graphic Packaging International, Inc. on or about February 5, 2015, at the address of 1171 W. Center St. Marion, OH: The employer failed to ensure the die and platen locking latch on the Cutter Machine (line #7202) were adequately guarded. Operating employees were exposed to amputations hazards while manually holding the cutting die and platen in place during lock down. When performing this task, operators hand(s) are within proximity of the mechanical locking arm of the die securing latch.
