29 CFR 1910.178(n)(8): Under all travel conditions the truck was not operated at a speed that would permit it to be brought to a stop in a safe manner. (a) On or about January 29, 2015; the G3605 Cat propane forklift was transporting a basket of parts, when the brakes were applied, the basket fell off the forks striking a cart on the floor exposing employee to being caught between and struck-by hazard.

29 CFR 1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways or wherever turns or passage had to be made: (a) On or about January 29, 2015; in the weld area, a safe path of travel was not provided by Line 3 when the forklift operator operating the G3605 Cat propane forklift was maneuvering out to the aisle way. This exposed employees to struck-by hazards.
