29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. On or about May 19, 2020, employees operating the Cincinnati 400 FM x 16 FT Press Brake with a 400 Ton Max Capacity, Serial # 44877, were not protected from pinch point hazard created as metal components were bent and pressed against the front side of the forming die.

29 CFR 1910.252(b)(2)(iii): Protection from arc welding rays. Where the work permits, the welder should be enclosed in an individual booth painted with a finish of low reflectivity such as zinc oxide (an important factor for absorbing ultraviolet radiations) and lamp black, or shall be enclosed with noncombustible screens similarly painted. Booths and screens shall permit circulation of air at floor level. Workers or other persons adjacent to the welding areas shall be protected from the rays by noncombustible or flameproof screens or shields or shall be required to wear appropriate goggles. On or about June 16, 2020, employees working and moving adjacent to welding stations in the welding department were not protected from harmful UV rays by welding screens or goggles.
