29 CFR 1910.132(a): Protective equipment was not provided when necessary whenever hazards capable of causing injury and impairment were encountered: (a) Batch Rooms: On or about 2/10/15, employees were not provided with protective equipment for fall protection when performing maintenance work on top of the Batch Tanks exposing them to a fall hazard of approximately nine feet to the concrete floor below.
