29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace: On or about July 25, 2023, at 500 Pepsi Place, Youngstown, OH 44502, the employer failed to ensure employees operating forklifts including but not limited to, Toyota (propane powered) Forklift Truck, model number 8FGCU20, Serial number 34418, were fully trained prior to allowing them to lift pallets, skids and other material in and around the warehouse.
