29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired.  a. At the worksite where employees used cords and equipment connected by cord and plug, an inspection of the equipment before each days use was not conducted, thereby exposing employees to an electrical contact hazard.

At the worksite where employees used a DeWalt radio charger as a receptacle for extension cords, the ground prong was missing, thereby exposing employees to a 126 volt electrical contact hazard.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables shall be protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage.  a. At the worksite where employees used an extension cord to provide power to a radio charger, the cord was ran across the driveway without protection resulting in damage to the cord, thereby exposing employees to a 126 volt electrical contact hazard.
