29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws: a. On or about August 2, 2016, at the Tuller Flats Apartments site, building 1, strain relief was not provided on the plug end of the flexible cord on the Milwaukee Heavy Duty Super Hawg 1/2 inch drill (CAT #1680-20) which caused damage to the outer insulation of the cord.
