29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers. a. In the main hallway where employees were installing plumbing pipes, protection such as a cover was not placed over the floor hole and secured in place thereby exposing employees to a trip/fall hazard.
