29 CFR 1926.405(g)(2)(iii): The repair splice in a hard service flexible cord No. 12 or larger did not retain the insulation, outer sheath properties, and usage characteristics of the cord which was being used: a. On February 13, 2015, employees were using a #12 extension cord with cuts in several places on the outer insulation and had been wrapped in black electrical tape, to operate a 120 VAC Ju-Shin paint mixer.
