29 CFR 1910.1200(e)(1): Employers did not develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training were met (Referenced in 29 CFR 1926.59):    a.  On February 13, 2015, a written hazard communication program was not developed for employees that worked with chemicals to include, but not limited to masonry primer, concrete paint, thin-set tile adhesives, and tile grout.

29 CFR 1926.405(g)(2)(iii): The repair splice in a hard service flexible cord No. 12 or larger did not retain the insulation, outer sheath properties, and usage characteristics of the cord which was being used:   a. On February 13, 2015, employees were using a #12 extension cord with cuts in several places on the outer insulation and had been wrapped in black electrical tape, to operate a 120 VAC Ju-Shin paint mixer.
