29 CFR 1926.403(i)(2)(i): Except as required or permitted elsewhere in this subpart, live parts of electric equipment operating at 50 volts or more was not guarded against accidental contact by cabinets or other forms of enclosures, or by any of the following means: a) On February 17, 2015, in the foyer area, there was no cover on the 24 inch high by 14 inch wide electric service entrance panel.

29 CFR 1926.51(f)(1): The employer did not provide adequate washing facilities for employees engaged in the application of paints, coating, herbicides, or insecticides, or in other operations where contaminants may be harmful to the employees. Such facilities were not in near proximity to the worksite and were not so equipped as to enable employees to remove such substances: a) On February 17, 2015, employees rolled Kilz 2 Latex paint on the walls and the employer did not provide soap or means to dry off, such as, hand towels or powered dryers.
