On or about March 23, 2015, in the following locations, flights of stairs having four or more risers were not equipped with standard railings, exposing employees to fall hazards: 1. Multi-Pack Department: Two flights of stairs leading to a rooftop penthouse were missing a mid rail on both sides. 2. Paper Machine Area: Two flights of stairs leading to Tissue Machine TM-1 were missing a mid rail on both sides.
