29 CFR 1915.89(e)(2): The employer did not use an orderly shutdown to prevent exposing any employee to risks associated with hazardous energy. F/V Unimak, Kloosterboer dock, Factory area: On or about 10 February 2015 and times prior thereto, The two hydraulically operated fish doors on the fish tanks were not being locked out when employees are accessing the tank. This condition exposed employees to a struck by hazard.

29 CFR 1918.35: Employees were working around open weather deck hatches which were not protected by coamings to a height of 24 inches (0.61 m), or by taut lines or barricades at a height of 36 to 42 inches (0.91 m to 1.07 m) above the deck. F/V Unimak, Kloosterboer dock, freezer deck hatch: On or about 10 February 2015 and times prior thereto, the rope railing on one corner of the hatch measured 27 inches. this condition exposed employees to a 12 foot fall hazard.
